Your terms (kept on this device)

Our program: child care agreement

Who this agreement is between

This agreement is between ________, who runs ________ (“the program”), and the parent or guardian who signs below, for the care of the child named below.

Hours

The program is open Monday to Friday, 7:00 am to 5:30 pm.

Children are picked up by 5:30 pm. Only the grown-ups named on the child's pick-up list may take the child home, and we may ask to see identification.

Fees and payment

Tuition is $______ a week, due every Monday, for the week ahead.

Tuition holds the child's place, so it is the same whether or not the child attends on a given day.

We will give at least 4 weeks' written notice before tuition changes.

Closed days and vacations

The program is closed on New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day and the day after, Christmas Eve and Christmas Day.

The provider takes one week in summer and one week at the end of the year, with a month's notice.

When a family takes a vacation, tuition is paid as usual, because the child's place is held.

When a child stays home

A child stays home with fever, vomiting or diarrhea in the last 24 hours, a rash with a fever, or any illness our state or local health department says keeps a child home.

We will call you to pick up a child who becomes sick during the day. Please keep your phone numbers up to date on the enrollment form.

Meals and what to bring

We provide meals and snacks.

Please bring a full change of clothes, diapers and wipes if used, a blanket for rest time, labeled with your child's first name.

Ending care

Either side may end this agreement with 2 weeks' written notice. Tuition is due for the notice period.

We may end care sooner if a child's safety, or other children's, is at risk.

How to use it

  1. Fill in your terms. Leave out anything you don't charge.
  2. Pick a contract, the parent handbook or a letter.
  3. Print it, read it through, and have a lawyer review it before families sign.

Ideas for class

  • Write tuition as a fee for holding the place, not for each day attended: it's what keeps a home program steady.
  • Give a month's notice before a tuition change, and put the reason in one line.
  • Hand the handbook out with the enrollment forms, and keep the signed agreement with them.

Questions

Is this a legal contract?

It is a starting draft in plain words, not legal advice. Rules on contracts, fees and notice differ by state. Check your state's child care rules and have a lawyer review it before you use it.

Where do my terms go?

They stay in this browser, so the next letter is quick. The child's name is a blank line on the paper; nothing about a child is typed here.

What should a daycare contract include?

Who it is between, hours, tuition and when it is due, late fees, closed days and vacations, when a child stays home sick, what to bring, and how either side ends care, with signatures.

Nothing you type here is sent to us. Your terms stay in this browser; no child's details are typed. No account, no ads.
