Sub feedback form: one fair page
A short form for the teacher who was out to fill in the morning after: how much of the plans got done, what the note said, the work and the room, anything that happened, and what you'll leave next time. It asks about the day, not for a score of the person.
Thank you for covering. This is what I found when I got back, so we can make the next day go smoothly. It is about the day, not a score.
To: ____________________ From: ____________________ Room: ________
No students' full names on this form. First names or initials only, if you need them at all.
How to use it
- Print a few and keep them in your sub folder.
- The morning you're back, fill one in from the sub's note and what you find.
- Give it to the sub or the sub office, and keep the last answer for your own next plans.

Questions
What should a substitute teacher evaluation form ask?
What happened, not what the sub is like: how much of the plans got done, whether the note covered what was done and who was absent, whether the work was collected, anything that happened, and what went well. Questions about events are fairer and more useful than a rating.
Why are there no stars or ratings?
A score says little about a day the teacher didn't see, and plans that ran short are often the plan's fault. Facts about the day help the sub, the office and your next plans.
Should I write students' names on it?
No full names. Use first names or initials if you need them at all; the form goes to people outside your room.
